I downloaded the file, the GDS seems correct. So it must be that the old
index filename.gbx wasn't deleted.

Looks like the attribute 'grid_spacing' in step 1 above is not set
properly.
It says '0.0 degrees_east', while it should be '0.25 degrees_east'.
